Although Ballance had the manuscript of this work ready for publication prior to the beginning of World War I, his service with the Army caused him to postpone its publication until war's end. Ballance developed the treatise from papers written over a twenty-five year period, extensively rewriting and expanding the papers for this book. Each chapter covers a separate aspect of the subject, and the references that accompany each were compiled by Charles David Green (fl. 1900), a colleague and fellow physician.
