Ewald was an active member of the Berlin Medical Faculty having qualified as a lecturer in internal medicine in 1874, extraordinary professor in 1883, and having been made an honorary professor in 1909. In the Foreword to this second edition, he commented that many advances had been made in what was known about thyroid diseases since the first edition was published thirteen years earlier. In addition, more was known about therapeutic approaches to these diseases and Ewald incorporated all this new knowledge in this edition. He also greatly expanded the already extensive bibliography from the first edition.
