Bigelow first published the present work in the American journal of the medical sciences (see No. 1863). It is here in book form for the first time under a changed title and with some expansion of the text. The text represents that which appeared in the January 1878 issue of the American journal of the medical sciences, the June 8, 1878 issue of the New York medical record, and also includes some additions by the author.
