In the Preface to this work, written at Churchill's Dublin home, he comments that "it is with much gratification that I acknowledge this volume to owe its existence to the solicitations of my excellent American publishers." Although the individual's name is not mentioned, Lea and Blanchard selected an American physician to compile the index and prepare a bibliography of the authors and works cited in the text.
